dc.descriptionEl presente trabajo de investigación tuvo como objetivo principal, la identificación de aquellos factores pronósticos que influyen en la mortalidad de pacientes diagnosticados con cáncer de mama, con base en factores ya preestablecidos, analizando su significancia e influencia directa; así como relaciones que permitan generar modelos predictivos que anticipen el comportamiento de la enfermedad en un paciente. El desarrollo del trabajo se realizó en el Instituto Oncológico Nacional “Dr. Juan Tanca Marengo” (SOLCA Matriz Guayaquil), con base en los datos que reposan en el sistema propio de la institución. Se tomó en cuenta a todos los pacientes diagnosticados con cáncer de mama en el periodo 2015-2020, teniendo un total de 3062 pacientes, trabajando únicamente con 956 de ellos luego de una correcta depuración de datos. Se realizó una investigación mixta, combinando variables de carácter cualitativo y cuantitativo, estudiadas a través de modelos predictivos modelados a partir de regresión logística misma que se realizó en el software estadístico R. Además, es de carácter teórico, abarcando una investigación descriptiva y explicativa, de índole no experimental, basándose en una inferencia hipotética-deductiva. Con un 95% de confianza se plantearon tres modelos, que resultaron significativos; con una capacidad de precisión para predecir aceptable (mayor al 75%), definiendo los tres modelos como factores pronósticos influyentes al tamaño, peso, tiempo de evolución, tamaño tumoral, estadio, y los estados que conforman la estadificación (T, M, N). Además, con probabilidades aproximadamente aceptables se define al modelo uno, como el más idóneo para pronosticar el comportamiento de un paciente ante la enfermedad. El estudio de factores pronósticos es un tema importante a tratar a un futuro con un mayor número de pacientes, con el fin de conseguir modelos más precisos y con una mayor capacidad predictiva, que nos permitan delimitar factores pronósticos más influyentes y decisivos.es_ES
dc.description.abstractThe main objective of this research work was to identify those prognostic factors that influence the mortality of patients diagnosed with breast cancer, based on pre-established factors, analyzing their significance and direct influence; as well as relationships that allow generating predictive models that anticipate the behavior of the Disease in a patient. The development of the work was carried out at the National Oncological Institute “Dr. Juan Tanca Marengo” (SOLCA Guayaquil), based on the data that rest in the institution´s own system. All patients diagnosed with breast cancer in the period 2015-2020 were taken into account, with a total of 3062 patients, working only with 956 of them after correct data filtering. A mixed investigation was carried out, combining qualitative and quantitative variables, studied through predictive models modeled from the same logistic regression that was carried out in the statistical software R. In addition, it is of a theoretical type, encompassing a descriptive and explanatory investigation, of a non-experimental nature, based on a hypothetical-deductive inference. With 95% confidence, three models were proposed, which were significant; with an acceptable accurate ability to predict (greater than 75%), defining the three models as prognostic factors influencing size, weight, time of evolution, tumor size, stage, and the states that form the stating (T, M, N). In addition, with approximately acceptable probabilities, model one is defined as the most suitable for predicting the behavior of a patient before the disease. The study of prognostic factors is an important topic to deal with in the future with a bigger number of patients, in order to achieve more precise models with a greater predictive capacity, which allow us to define more influential and decisive prognostic factors.es_ES
