dc.descriptionSe evaluó el impacto de la aplicación de seguimiento farmacoterapéutico en pacientes de consulta externa con diabetes mellitus e hipertensión arterial del Hospital Andino Alternativo de Chimborazo de la ciudad de Riobamba, empleándose como estrategias el Método Dáder para identificar los Problemas Relacionados a Medicamentos (PRM) y los Resultados Negativos Asociados con la Medicación (RNM) mediante intervenciones farmacéuticas, y utilizando el Test Morisky-Green-Levine para medir la adherencia del tratamiento farmacológico. En el estudio participaron 24 pacientes; 58% mujeres y 42% hombres, con rango 41- 90 años, en los cuales se registran 126 (RNM); 84 casos relacionados con efectividad, 27 con necesidad y 15 con seguridad. Los (PRM) encontrados para casos no prevenibles corresponden al 11.11% y los prevenibles al 88.88%: el 62.4% para inefectividad cuantitativa, 11.6% problema de salud no tratado, 7.1% efecto de medicamento innecesario, 6.3% inefectividad no cuantitativa, 6.3% inseguridad no cuantitativa y 6.3% inseguridad cuantitativa, siendo más frecuente las interacciones medicamentosas (hipoglucemiantes y antihipertensivos- agentes antiinflamatorios no esteroideos) y con plantas medicinales (manzanilla). Con respeto a la adherencia del tratamiento farmacológico, el 79% de la población no fueron adherentes al inicio, mientras que al final la adherencia fue 92%. Mediante el test student se determinó la estabilidad clínica de parámetros: glucemia con promedio final de 129 mg/dL siendo estadísticamente significativo en 20mg/dL; presión arterial un promedio 123.9mmHg para la sistólica con diferencia significativa de 7mmHg y 73.7 mmHg para la diastólica sin diferencia con 95% de confianza. Se concluye la obtención de un impacto positivo mediante la ejecución de estrategias farmacéuticas que permiten mantener estable el control de las patologías y por ende mejorar la calidad de vida del paciente. Se sugiere a gerencia del hospital que propicie la integración del equipo de salud para trabajar reforzando continuamente la educación sanitaria sobre estas medidas correctivas y preventivas.es_ES
dc.description.abstractThe impact of the application of pharmaco-terapeutic monitoring in outpatients with diabetes myelitis and hypertension, was evaluated at the Andean Alternative Chimborazo Hospital in Riobamba city. It was evaluated using the DADER method as strategy to identify problems related to PRM drugs and negative outcomes associated with RNM medication trough pharmaceutical interventions, using the test Morisky-Green-Levine to measure adherence to pharmacological treatment. In the study participated 24 patients, 58% female and 42% male in the range of 41 to 90 years old, 126 RNM registrants, 84 cases related with effectiveness, 27 with need and 15 with security. The PRM founded for non-preventable cases correspond to 11.11% and preventable to 88.88%, 62.4% for quantitative ineffectiveness, 11.6% with untreated health problem, 7.1% effect of unnecessary medication, 6.3% non-quantitative ineffectiveness, 6.3% no quantitative insecurity and 6.3% quantitative safety, the most common are the drug interactions (hypoglycemic and anti-hypertensive, non-steroidal anti-inflammatory agents) and medicinal plants (chamomile). With regard to adherence to drug treatment, 79% of the population were non-adherent at the beginning, while at the end the adherence was 92%. By Student test, the clinical stability parameters were determined: blood glucose with final average of 129 mg/dL, it is statistically significant at 20mg/dL, blood pressure an average of 123.9 mmHg for systolic with significant difference of 7 mmHg for diastolic no difference with 95% of confidence. It is concluded the obtaining of a positive impact by executing pharmaceutical strategies to maintain stable the control of diseases and thus improve the quality of life patients. It is suggested to Hospital management to encourages the integration of the health team to work continuously strengthening health education on these corrective measures.es_ES
