dc.descriptionSe implementó un sistema de seguimiento farmacoterapéutico continuo en el hospital del Instituto Ecuatoriano de Seguridad Social “IESS”, para la ciudad de Riobamba. El estudio se aplicó en pacientes portadores de hipertensión arterial, diabetes mellitus e hiperlipidemia; la captación se realizó en el área de consulta externa de cardiología, y fueron elegidos de forma aleatoria, bajo criterio de aceptación voluntaria. En la investigación se analizó el estado de salud del paciente a través de la historia clínica; se evaluó las necesidades relacionadas con medicamentos mediante entrevistas, encuestas, perfiles farmacoterapéuticos; se elaboró planes de cuidado en lo que respecta a administración correcta de medicamentos, cumplimiento, alimentación, actividad física, entre otros. Además se realizó mediciones periódicas de presión arterial, glucosa basal, glucosa postprandial, hemoglobina glicosilada, colesterol, triglicéridos, LDL, HDL; determinando un mejor control luego de realizar el seguimiento farmacoterapéutico. En el estudio se identificó 305 resultados negativos asociados con la medicación; de los cuales el 35.4% se deben a inseguridad no cuantitativa, el 30.8% a inseguridad cuantitativa, y el 24.8% a inefectividad cuantitativa; los principales problemas relacionados con medicamentos en el grupo de pacientes fueron por efectos adversos, sinergismo, y/o antagonismo con 35.4%, 28.1%, y 15.4% respectivamente, el 21.1% de RNM pueden ser evitados o corregidos, el 78.9% de RNM no pueden ser evitados, y el 64.6% de RNM se producen por falla humana; entre los cuales el 52.4% se deben a problemas ocasionados por el prescriptor y el 12.2% por el paciente. La implementación del sistema de atención al paciente contribuyó benéficamente en su salud; se recomienda que el método continúe aplicándose en el hospital del IESS Riobamba, y se implemente sistemas similares en otros centros de salud.es_ES
dc.description.abstractA continuous pharmacotherapeutic follow-up system was implemented in the hospital of the Instituto Ecuatoriano de Seguridad Social "IESS" (Ecuadorian Institute of Social Security) in the city of Riobamba. The study was applied to patients with arterial hypertension, diabetes mellitus and hyperlipidemia; the patients were recruited in the cardiology outpatient area and were randomly selected under voluntary acceptance criteria. During the investigation, the patient's health status was analyzed through the clinical history; medication-related needs were evaluated by means of interviews, surveys, pharmacotherapeutic profiles; care plans were drawn up regarding correct administration of medications, compliance, diet, physical activity, among others. In addition, periodic measurements of blood pressure, basal glucose, postprandial glucose, glycosylated hemoglobin, cholesterol, triglycerides, LDL, HDL were taken, determining better control after pharmacotherapeutic follow-up. The study identified 305 negative results associated with medication; of which 35.4% were due to non-quantitative insecurity, 30.8% to quantitative insecurity, and 24.8% to quantitative ineffectiveness; the main drug-related problems in the group of patients were due to adverse effects, synergism, and/or antagonism with 35. 4%, 28.1%, and 15.4% respectively, 21.1% of RNM can be avoided or corrected, 78.9% of RNM cannot be avoided, and 64.6% of RNM are caused by human failure; among which 52.4% are due to problems caused by the prescriber and 12.2% by the patient. The implementation of the patient care system contributed beneficially to patient health; it is recommended that the method continue to be applied in the IESS Riobamba hospital, and that similar systems be implemented in other health centers.es_ES
